4. Record-Keeping (Logging)

High-risk AI systems must automatically log events enabling traceability. What was decided? When? Based on what inputs? Who was affected? This supports accountability and enables investigating incidents or complaints.

5. Transparency and Information Provision

Users must receive clear, comprehensive information about the AI system's capabilities, limitations, level of accuracy and robustness metrics, purpose and intended use, risks and appropriate oversight measures, and contact information for inquiries or complaints.

6. Human Oversight

High-risk AI must operate under meaningful human supervision. This means humans can understand system outputs, monitor operation in real-time, intervene or override decisions when necessary, and remain aware of risks of automation bias.

"Rubber-stamping" AI outputs doesn't constitute oversight. Human supervisors must have authority, competence, and resources to genuinely control the AI system.

7. Accuracy, Robustness, and Cybersecurity

Systems must achieve appropriate levels of performance accuracy, resilience to errors or attacks, and security against unauthorized access or manipulation. The required rigor scales with risk—medical diagnostic AI faces higher standards than inventory forecasting.

8. Conformity Assessment

Before market release, high-risk AI systems must undergo conformity assessment verifying compliance. Depending on the system type, this may involve internal checks by the provider or third-party assessment by notified bodies.

Upon successful assessment, providers affix the CE marking and register the system in the EU database for high-risk AI.

The Act introduces specific rules for foundation models like GPT-4, Claude, or Llama that can be adapted for numerous tasks. Providers of GPAI models must prepare technical documentation, provide information to downstream deployers, implement copyright compliance policies, and publish training data summaries.

For models with "systemic risk" (extremely powerful systems), additional requirements apply including adversarial testing, serious incident tracking and reporting, cybersecurity protections, and energy efficiency reporting.
